2011 Region II ODP Holdover Payment

The Holdover Payment Form for each player attending a holdover camp is available below. Players from Kentucky, Minnesota, Missouri, Nebraska and Wisconsin are not required to submit the form below as those states. Iowa, Illinois, Indiana, Michigan, Ohio South, Ohio North and Dakota ODP are required to fill out the form and enclose the $250 Cost for camp.
